Vocational Support Service (DHB)
(Services are provided throughout the Waikato and Taranaki District Health Board regions).
Includes employment and education support that is community based and provides service users with the support, training and assistance necessary for them to gain employment.
The service provides flexible, ongoing support to help you maintain your vocational goals. You can have as much responsibility as you want for determining the content of the programme and the day-to-day running of the service.
The style of our service is that:
- There is an emphasis on supporting your strengths and those of your family/whanau and significant others;
- Our vocational support team will work with you to reach desirable outcomes;
- The particular needs of Maori and Pacific Peoples service users are met by the provision of culturally derived skills programmes and activities;
- Wherever possible, relevant staff will be of Maori or Pacific Island descent.
- Community Support Facilitators
A Community Support Facilitator receives your referral, arranges to meet with you and supports you to develop a plan that will help you reach your goals. They also work with you to plan suitable activities that will help you reach your goals.
Entry Criteria: You can access this service if you are eligible under the Ministry of Health's guidelines for the provision of health and disability support services. If you are not sure, please contact Progress to Health, Freephone: 0800 77 57 57 for assistance.
Access: We welcome referrals directly from a potential service user, GP's, other health professionals, community mental health services, or community support services and family/whanau.
Cultural support can be available e.g. access to Kaumatua or interpreters.
